These Terms govern your access to and use of the Infusion Sciences website, online forms, appointment systems, patient communication tools, and related digital services.

In these Terms, “Infusion Sciences,” “we,” “us,” and “our” refer to Infusion Sciences and, where applicable, its affiliated medical practices, licensed healthcare providers, employees, contractors, and service providers.

By visiting infusionsciences.com, submitting information, requesting an appointment, creating an account, purchasing a service, or otherwise using our website or services, you agree to these Terms.

Please do not use our website or services if you do not agree to these Terms.

1. Eligibility

You must be at least 18 years old and legally capable of entering into a binding agreement to use our services independently.

Services for a minor may only be requested by a parent, legal guardian, or another person legally authorized to make healthcare decisions for the minor, and only when the requested service is legally and clinically appropriate.

By using our website or services, you represent that:

  • The information you provide is accurate and complete
  • You are legally authorized to provide the submitted information
  • You will use the services only for lawful purposes
  • You are not impersonating another individual
  • You are physically located in a jurisdiction where the applicable healthcare provider is authorized to provide services at the time of a telehealth appointment

Availability of services may depend on your age, medical history, state of residence, physical location, provider licensure, clinical eligibility, and applicable law.

2. Website Information Is Not Medical Advice

The content available through the Infusion Sciences website is provided for general educational and informational purposes.

Website content may discuss health, wellness, longevity, recovery, hormones, metabolic health, peptides, medications, laboratory testing, or other healthcare topics. This information is not intended to:

  • Diagnose a medical condition
  • Replace an examination by a qualified healthcare professional
  • Establish a provider-patient relationship
  • Recommend a particular medication or treatment for you
  • Replace professional medical advice
  • Guarantee that any treatment is appropriate, safe, or effective for you

Do not disregard professional medical advice or delay seeking care because of information you viewed on our website.

A provider-patient relationship is established only after all required registration and consent steps have been completed and a licensed healthcare provider has agreed to evaluate or treat you.

3. Medical Emergencies

Infusion Sciences does not provide emergency medical services through its website, email, text messaging, contact forms, or routine telehealth services.

Do not use these services for an urgent or life-threatening medical condition.

If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency department immediately.

Do not wait for an email, telephone call, text message, appointment confirmation, or website response from Infusion Sciences.

4. Telehealth Services

Telehealth may involve the delivery of healthcare services through video, audio, telephone, secure messaging, electronic forms, remote monitoring, photographs, or other communications technology.

Telehealth has potential benefits, including increased convenience and access to care. It also has limitations and risks, including:

  • Technology failures or interruptions
  • Delays in communication

A provider may determine at any time that telehealth is not appropriate for your condition. You may be asked to obtain laboratory testing, provide additional records, visit an in-person healthcare provider, seek urgent care, or go to an emergency department.

Telehealth availability is not guaranteed in every state or for every medical condition.

5. Independent Clinical Judgment

All medical decisions are made by licensed healthcare professionals using their independent professional judgment.

Submitting a form, completing an intake questionnaire, making a payment, requesting a medication, or scheduling an appointment does not guarantee that:

  • You will be accepted as a patient
  • You are eligible for a particular treatment
  • A provider will issue a prescription
  • A requested medication will be prescribed
  • A prescription will be renewed
  • A pharmacy will dispense a medication
  • A laboratory test will be ordered
  • A particular outcome will be achieved

A provider may decline, discontinue, modify, or recommend an alternative treatment when clinically appropriate or legally required.

Infusion Sciences does not interfere with the independent medical judgment of licensed healthcare providers.

6. Your Health Information and Responsibilities

You agree to provide complete, accurate, and current information regarding your identity and health, including:

  • Symptoms and health concerns
  • Medical and surgical history
  • Current medications and supplements
  • Allergies and adverse reactions
  • Existing diagnoses
  • Pregnancy or breastfeeding status where relevant
  • Laboratory results
  • Substance use where clinically relevant
  • Treatments received from other healthcare providers
  • Changes in your health or medication use

You are responsible for promptly updating information that becomes inaccurate or incomplete.

Withholding or misrepresenting material health information may result in inappropriate care, delayed treatment, adverse effects, termination of services, or other harm.

You agree to follow instructions provided by your healthcare professional, including instructions relating to medication use, laboratory monitoring, follow-up appointments, side effects, storage, and emergency care.

You should inform your primary care provider and other treating professionals about medications, supplements, or treatments obtained through Infusion Sciences.

7. Prescriptions and Medications

A prescription will only be issued when a licensed provider determines that it is clinically appropriate and legally permissible.

Prescription decisions may depend on:

  • Your medical history
  • Current symptoms
  • Laboratory results
  • Medication interactions
  • Contraindications
  • State and federal prescribing laws
  • Provider licensure
  • Pharmacy availability
  • Required follow-up care

You agree to use medication only as directed and only for the individual for whom it was prescribed.

You may not sell, share, transfer, misuse, or distribute medication obtained through our services.

You must promptly report serious or unexpected side effects to an appropriate healthcare professional. Seek emergency care immediately when symptoms may represent a medical emergency.

Infusion Sciences does not guarantee the continued availability, pricing, formulation, manufacturer, or pharmacy source of any medication.

8. Off-Label and Compounded Medications

A licensed provider may, where clinically appropriate and legally permitted, discuss or prescribe a medication for a use that is not included in its FDA-approved labeling. This is commonly referred to as off-label use.

Some treatment plans may also involve medications prepared by a compounding pharmacy.

Compounded medications are not FDA-approved. The FDA does not review compounded medications for safety, effectiveness, or quality before they are marketed.

When compounded or off-label medication is proposed, you may receive additional disclosures, medication guides, informed-consent documents, or pharmacy information. You should carefully review this information and ask your provider any questions before beginning treatment.

Nothing on the website should be interpreted as representing that a compounded medication is identical or equivalent to an FDA-approved brand-name or generic medication.

9. Laboratory Testing

Some services may require laboratory testing before treatment begins or while treatment continues.

You may be responsible for:

  • Scheduling or completing required testing
  • Following preparation instructions
  • Paying laboratory fees
  • Providing previous test results
  • Completing follow-up testing within the requested period

Laboratory services may be provided by independent third parties. Infusion Sciences does not control every aspect of an independent laboratory's scheduling, billing, testing procedures, processing times, or reporting systems.

Laboratory results must be interpreted by a qualified healthcare professional. A result displayed through a portal or communicated electronically should not be interpreted without appropriate clinical guidance.

Failure to complete required testing may result in delayed, modified, suspended, or discontinued treatment.

10. Pharmacies, Shipping, and Third-Party Fulfillment

Medications, products, or supplies may be fulfilled by independent pharmacies, laboratories, manufacturers, carriers, or other third parties.

Delivery dates are estimates and are not guaranteed.

Delays may occur due to:

  • Prescription review
  • Pharmacy processing
  • Product availability
  • Weather
  • Carrier delays
  • Address errors
  • Regulatory requirements
  • Additional information requests
  • Events outside our reasonable control

You are responsible for providing an accurate delivery address and promptly reporting delivery problems.

Certain medications may require refrigeration, temperature control, special handling, or immediate storage. You are responsible for following all delivery and storage instructions.

Infusion Sciences is not responsible for loss or damage caused by your failure to retrieve, inspect, refrigerate, store, or use a delivered item properly.

Any issue involving damaged, missing, delayed, or temperature-sensitive medication should be reported promptly using the contact information provided with your order.

11. Appointments

You are responsible for attending scheduled appointments on time and ensuring that:

  • Your device is functioning
  • Your internet or telephone connection is adequate
  • You are in a private and appropriate location
  • You are physically located in the state disclosed during scheduling
  • You have access to any requested records, medications, or laboratory results
  • You are not driving or operating machinery during the consultation

We may reschedule, delay, or cancel an appointment because of provider availability, emergencies, technology issues, licensing restrictions, incomplete forms, missing laboratory results, or other operational or clinical reasons.

Appointment availability does not guarantee treatment eligibility.

12. Fees and Payment

You agree to pay all fees disclosed before purchasing or receiving a service.

Fees may include:

  • Consultation fees
  • Membership or program fees
  • Laboratory fees
  • Medication costs
  • Pharmacy or fulfillment fees
  • Shipping charges
  • Follow-up appointment fees
  • Administrative fees
  • Other charges disclosed during checkout or enrollment

Unless expressly stated otherwise, Infusion Sciences does not guarantee that services, consultations, laboratory tests, medications, or products will be covered or reimbursed by insurance.

You are responsible for determining whether your insurance or benefit plan will provide coverage or reimbursement.

You authorize Infusion Sciences and its payment providers to charge the payment method you submit for amounts you approve.

You represent that you are authorized to use the payment method provided.

13. Recurring Programs and Subscriptions

A recurring program or subscription will apply only when recurring billing is clearly disclosed and you expressly agree to it.

Before enrollment, applicable checkout or program materials should disclose:

  • The amount or method used to calculate the charge
  • The billing frequency
  • What is included
  • Any minimum commitment
  • How to cancel
  • When cancellation becomes effective

By enrolling in a recurring program, you authorize charges according to the terms presented during enrollment.

You are responsible for maintaining accurate billing information.

Cancellation of a recurring program does not automatically reverse charges already processed, services already delivered, appointments already completed, medications already ordered, or costs already incurred.

Clinical care may need to continue temporarily for safety reasons even after a membership or administrative program has been canceled. Your provider will determine whether follow-up care or medication instructions are medically necessary.

14. Cancellations, Rescheduling, and Refunds

Appointment cancellation, rescheduling, no-show, product, program, and refund rules may be provided during booking, enrollment, checkout, or purchase.

You agree to review the applicable policy before completing a transaction.

Unless required by law or otherwise stated in a specific written policy:

  • Completed consultations are not refundable
  • Fees for services already performed are not refundable
  • Laboratory fees may become nonrefundable once an order is submitted
  • Medication fees may become nonrefundable once a prescription is processed or sent for fulfillment
  • Customized, compounded, temperature-sensitive, or prescription products may not be returnable
  • Shipping and fulfillment charges may not be refundable
  • Refunds do not guarantee cancellation of clinical follow-up obligations

Infusion Sciences may issue a refund or account credit at its discretion when appropriate. Providing a refund in one situation does not create an obligation to provide the same result in another situation.

Nothing in this section limits rights that cannot legally be waived.

15. Communication Consent

By providing your telephone number or email address, you authorize Infusion Sciences and its service providers to contact you concerning:

  • Appointment requests
  • Registration
  • Intake requirements
  • Treatment-related information
  • Laboratory testing
  • Prescriptions
  • Orders and delivery
  • Billing
  • Account security
  • Customer support
  • Follow-up care

Communications may be sent through telephone calls, voicemail, email, text message, or secure electronic messaging.

Message and data rates may apply.

You may separately consent to promotional communications. Consent to receive promotional text messages is not a condition of receiving healthcare services.

Opting out of promotional messages will not prevent communications that are necessary for treatment, patient safety, transactions, account security, or legal compliance.

Standard email and text messages may not be fully secure. Do not send urgent or highly sensitive medical information through ordinary email or text messaging unless instructed to do so.

16. Privacy

Your use of the website and services is also governed by the Infusion Sciences Privacy Policy.

Health information handled by a HIPAA-covered healthcare provider or business associate may also be governed by an applicable Notice of Privacy Practices.

The Privacy Policy and Notice of Privacy Practices are separate documents with different purposes. If a conflict arises regarding Protected Health Information, the applicable Notice of Privacy Practices will control.

17. Accounts and Security

Some services may require you to create an account or access a patient portal.

You are responsible for:

  • Maintaining the confidentiality of your login credentials
  • Using a strong and unique password
  • Preventing unauthorized access to your device
  • Logging out of shared devices
  • Promptly reporting suspected unauthorized access
  • Keeping account information current

You may not access another person's account without legal authorization.

We may suspend access when we reasonably believe an account has been compromised, misused, or accessed unlawfully.

18. Acceptable Use

You may not use our website or services to:

  • Violate any law or regulation
  • Submit false, misleading, or fraudulent information
  • Impersonate another individual
  • Obtain medication for another person
  • Attempt to obtain medication through deception
  • Harass, threaten, or abuse staff, providers, patients, or contractors

24. Limitation of Liability

To the fullest extent permitted by law, Infusion Sciences will not be liable for indirect, incidental, special, consequential, exemplary, or punitive damages resulting from:

  • Your misuse, resale, sharing, or unlawful distribution of medication or products

This provision does not require you to indemnify a healthcare provider for the provider's own professional negligence or conduct that cannot lawfully be transferred to you.

26. Suspension or Termination

We may restrict, suspend, or terminate access to administrative or website services when:

  • These Terms are violated
  • Information is fraudulent or materially inaccurate
  • Payment obligations remain unpaid
  • Staff or providers are threatened or abused
  • Medication misuse or diversion is suspected
  • Continued service would violate law
  • A security risk exists
  • A provider determines that the available service is not appropriate

Termination of website or administrative access does not automatically terminate an established provider-patient relationship without appropriate notice and transition procedures where required by professional or legal obligations.

You remain responsible for charges incurred before termination.

27. Governing Law

These Terms are governed by the laws of the State of Florida, without regard to conflict-of-law principles.

Any dispute concerning the website or nonmedical administrative services must be brought in a state or federal court with appropriate jurisdiction in Florida, unless applicable law requires a different location.

Claims involving professional healthcare services may be subject to separate laws, procedures, consent documents, or agreements.

28. Severability

If any provision of these Terms is found invalid, illegal, or unenforceable, that provision will be enforced to the maximum extent legally permitted.

The remaining provisions will continue in full force and effect.

29. No Waiver

Our failure to enforce a provision of these Terms does not waive our right to enforce it later.

A waiver is effective only when made in writing by an authorized representative of Infusion Sciences.

30. Assignment

You may not transfer your rights or obligations under these Terms without our written permission.

We may assign these Terms as part of a merger, acquisition, restructuring, financing, sale of assets, change of ownership, or transfer to an affiliated organization.

31. Entire Agreement

These Terms, together with the Privacy Policy, applicable Notice of Privacy Practices, informed-consent forms, checkout terms, program terms, and other agreements presented to you, constitute the applicable agreement between you and Infusion Sciences concerning the relevant service.

If a service-specific agreement conflicts with these general Terms, the service-specific agreement will control for that service.

32. Changes to These Terms

We may update these Terms to reflect changes in our services, technology, business practices, or legal obligations.

Material changes may also be communicated through the website, email, account notification, or another appropriate method.

Your continued use of the website after updated Terms become effective means you accept the updated Terms, to the extent permitted by law.
